Problem With Modal Survey Plugin

Hi,

I’m trying to get this working but running into issues with how its being displayed.

Firstly the Admin interface is unuasble because when you mouse over the control icons they move around so you can’t click on them and generally just not displaying correctly.

When its live the confimation tick box does not display correctly on mobile devices making it difficult for customers to see what to do to complete the survey.

I have been in touch with the plugin auther and he suggested as it works fine on the default themes (I’ve confirmed this), I should ask here.

Can someone suggest what to do next?
