CheckBox and Radio Button Forms

I am having problems having a form with a checkbox appear on my site. I created a test page to see if it was something wrong with the Event Espresso plugin. http://fuelcincinnati.org/givebackcincinnati/test-page/

The problem is that the label appears, but the checkbox is nowhere to be found.

Please help, I have tried all sorts of stuff and can’t seem to figure this out.

hi, please open file bee/style.css and find line 51 comment item like this:/*-webkit-appearance: none;*/;
also file bee/styles/bbpress-mod.css and find line 1233 comment item like this:/*display: none;*/,

Are you certain it is line 1233? The line i have there is this:
.row-fluid .span9{width:74.30939226519337%;*width:74.25620077583166%;}

There should be checkboxes related to the payment types and after the agreement box.
