contact us form not working

Hi, we have place a contact form widget in the footer of our webpage, see http://www.myapplicationclinic.com. In widgets we simply added the one called Contact Form. But it doesn’t work, we receive no email when a message is sent from that form.
Our email smtp settings should be correct, since other contact forms on the site are working with email and sending test emails works fine.

Hi, I checked the wp-admin, I did not find the reason. There are always unpredictable problems in mail send function. You could try to use the contact form 7 plugin, use the text widget with shortcode of contact form 7

And it has nothing to do with the smtp – that the contact form widget in some way is not using this information or connected to this?

It seems odd that this contact form: http://www.myapplicationclinic.com/contact-us/ which is provided in the theme is working fine, but the one in the footer not. Cause that one is also part of the theme right?
